LPISD wants employees’ opinions on current health care benefits

LPISD is seeking the opinions of employees regarding the performance of the current health care provider (United Healthcare - UHC). If you are an employee of the district and receive district health benefits, please use the link below to complete a short survey (only 4 multiple choice questions). The survey results will be provided to the Health Benefits Committee and used to decide if LPISD will need to publish a request for proposals from other health care providers or continue with UHC.
Please complete the survey before January 11, 2008. If you change your mind, only the last survey that you submit will count. Thanks for your input.
This survey is intended only for LPISD employees receiving benefits from United Healthcare.
